This request form is to ensure background hour eligibility for our Master of Arts in Teaching programs. Upon submission, the Graduate Admissions Committee will review your request and unofficial transcripts to determine eligibility.
Please note: Applicants must meet program-specific eligibility requirements prior to being eligible to be hired or to receive an Option 6 letter.

PLEASE NOTE: If you are requesting a review for a Secondary (8-12) program, you will not be required to request a second Subject of Interest for Middle Grades (5-9) in the same subject. Those eligible for a subject area for (8-12) would already meet eligibility for the same (5-9) subject area.

30 hours of background in Math is required to pursue the MAT - Math (8-12) program or MAT - Middle Grades Math (5-9) program. Please note, if you are pursuing MAT - Middle Grades Math and are combining two middle grades emphasis areas, you will only need 25 hours.

Grade of C or higher
At the college level (100 or higher)
Courses in the following subject areas: Algebra; Geometry; Statistics; Methods and Materials for Teaching Math (3 hours max); CALCULUS IS REQUIRED (3 hour minimum)
May have a maximum of 9 hours in Accounting

Eligibility for a program does not grant automatic access to an Option 6 letter which is then used to apply for and obtain a teaching position while in the MAT program. Applicants must go through the appropriate admissions process for admittance and access to an Option 6 letter.
